We are excited about the opportunity to extend education into the world of work. Connecting with our industry partners allows students real world experiences in their desired industry sector. The information on this page will streamline the process of career related learning experiences, and enable easy access to the required paperwork to ensure safe, legal and professional partnerships between education institutions, industry partners, and students and their families.

General Forms for All Participants

• JPA-6 – Acknowledgement and Assumption of Potential Risk
  If a student is over 18 they will utilize the JPA 6 (non-student) form. This form will be utilized for Job Shadows as well.
• JPA-21(B) – Waiver for Student transportation in a non-district vehicle by a non-district person
  If non-district vehicles are utilized for job shadow and field trips this form will necessary as well
• HCOE Media Release Form
• Paid or Unpaid Internship Agreement Form
  • This form is to be signed by parents, students, school representative, and industry partners
  • As these training programs are an extension of the classroom experience, students participating in an unpaid Career Related Learning Experience will be provided Workman’s Compensation Insurance by the district
  • In a paid Career Related Learning Experience, Workman’s Compensation Insurance will be provided by the employer
